1. Senior Citizen's IDs
2. Voter's IDs
3. Digitized IDs from Government Agencies (e.g. SSS, PRC, BIR, Driver's License etc.)
4. School / Company ID
In case applicant has no ID from the list, he/she may submit documents with complete name, bithdate, birthplace and proof of citizenship.
Listed below are list of supporting documents you must bring if available:
1. Marriage Contract
2. Land Title
3. Driver's License
4. Government Service Record
5. School Form 137 or Transcript of Records (with readable dryseal)
6. Documents bearing full name, birth details and proof of citizenship
7. Voter's Registration Record
8. Baptismal Certificate (with readable dryseal)
9. Seaman's Book
10. Income Tax Return (old)
11. NBI Clearance
